Please read these Cordex Forms Terms & Conditions (Cordex Forms Terms) carefully. By purchasing, downloading, activating or using Cordex Forms, you agree to be bound by these Cordex Forms Terms and our general Terms & Conditions. If there is any inconsistency for matters specific to Cordex Forms, these Cordex Forms Terms prevail.
Acceptance
These Cordex Forms Terms form a binding agreement between you and My IT and Apps (ABN 82 659 633 888). If you are purchasing on behalf of an organisation, you warrant you have authority to bind it, and "you" includes that organisation.
The product
Cordex Forms is a software product (including a WordPress plugin and associated connectors) that captures submissions from WordPress forms and syncs leads, enquiries and contacts into Microsoft Dataverse and Dynamics 365, with no code required. Features and inclusions are those described on the Cordex Forms product page at the time of your purchase.
Licence grant
Cordex Forms is licensed, not sold. Subject to payment of the applicable fees and ongoing compliance with these Cordex Forms Terms, we grant you a non-exclusive, non-transferable, non-sublicensable, revocable licence to install and use Cordex Forms for your own internal business purposes, within the limits of your purchased tier.
Tiers & entitlements
Cordex Forms is offered in the following tiers. The exact site counts, connections, features and prices are those published at checkout at the time of purchase and govern your licence:

| Cordex Forms OEM / Reseller / Partner | Deploying, hosting, bundling, white-labelling or reselling Cordex Forms to or for third-party customers. Available only under a separate written agreement, priced per end-customer, per-tenant or by revenue share. |
You must not exceed the entitlements of your tier. If your usage grows beyond your tier, or you intend to make Cordex Forms available to third parties, you must upgrade or enter the appropriate agreement before doing so.
Purchase & checkout
Cordex Forms licences are purchased through our secure checkout provider. Prices are shown at checkout; local currency may be displayed for convenience, with conversion handled by the checkout provider. Unless stated to be inclusive, prices are exclusive of GST and applicable taxes, which are added where required by law. Your order is accepted when we issue your licence key or confirmation.
Subscription & renewal
Each licence includes a term (typically 12 months) of updates and support. Unless you cancel before the renewal date, or unless stated otherwise at checkout, licences renew for successive terms at the then-current price, and you authorise us or our checkout provider to charge the renewal fee. You can disable automatic renewal at any time by contacting us or via your account. If a licence lapses, updates and support cease and continued use without a current licence is not permitted.
Refunds
Nothing here limits your rights under the Australian Consumer Law. Subject to those rights, because Cordex Forms is delivered digitally we do not generally offer change-of-mind refunds once a licence key has been issued or the software downloaded. If Cordex Forms has a fault we cannot resolve within a reasonable time, contact us and we will work with you toward an appropriate remedy.
Licence keys & activation
Your licence key is confidential and issued for your use within your entitlements. You are responsible for keeping it secure. Sharing, publishing or reselling keys, or using keys beyond your entitlements, is a breach of these Cordex Forms Terms and may result in deactivation.
Use restrictions
- Do not copy, modify, reverse engineer, decompile or attempt to extract source code except to the extent permitted by law.
- Do not remove or alter any proprietary notices, branding or licensing mechanisms.
- Do not circumvent licence limits or security controls, or share licence keys.
- Do not use Cordex Forms to process data you are not entitled to process, or in breach of any law or third-party right.
- Do not resell, rent, lease, sublicense, host, bundle or otherwise provide Cordex Forms, or access to it, to or for any third party except under a separate written OEM/Reseller/Partner agreement with us.
Resale, OEM & redistribution
This clause prevails over any "unlimited sites" or white-label wording elsewhere. The Pro, Business and Enterprise tiers licence Cordex Forms for the licensee's own use: that is, for sites, environments and brands owned and operated by you (and, for Enterprise, your affiliated entities under common control), to process your own organisation's data.
Regardless of the number of sites permitted by your tier, you must not, without a separate written agreement with us:
- deploy, install, activate or operate Cordex Forms on behalf of, or for the benefit of, a third-party customer or client as a deliverable;
- host, offer or make Cordex Forms available to third parties as part of a product, platform, portal or managed/SaaS service;
- use white-label branding to distribute or on-sell Cordex Forms under your own or a client's brand; or
- redistribute, sublicense, resell, rent or lease Cordex Forms or its licence keys.
"Unlimited sites" on the Enterprise tier means unlimited sites within your own organisation and affiliated entities; it does not grant any right to serve, resell to, or deploy for third parties. Building solutions for clients, agencies delivering to customers, ISVs embedding Cordex Forms, and hosting providers require a Cordex Forms OEM / Reseller / Partner agreement, which we price per end-customer, per-tenant, per-deployment or by revenue share. Contact cordex-forms@myitandapps.com.au to arrange one. Activations may be registered and audited to enforce this clause.
Requirements & third parties
Cordex Forms requires a compatible WordPress installation, supported form plugin(s), and valid Microsoft licensing and environments (Dataverse / Dynamics 365, and where relevant Power Platform and Azure). You are responsible for obtaining and maintaining those third-party products, subscriptions and configurations. Your use of third-party products is governed by their own terms. We are not responsible for changes, deprecation, downtime or acts or omissions of third-party providers or your own environments.
Updates & support
During an active licence term we provide software updates and reasonable email support, aiming to respond within one business day (Monday–Friday, excluding Queensland public holidays). Support covers the Cordex Forms software as supplied and does not extend to your third-party environments, custom code, or unsupported configurations.
Data handling
Cordex Forms processes form submissions and related records that you direct it to sync. You retain all rights in that data and are responsible for having the necessary rights and consents to collect and process it, and for configuring the product in line with your legal obligations (including the Privacy Act 1988 (Cth) and the Australian Privacy Principles). Our handling of personal information is described in our Privacy Policy.
Intellectual property
All intellectual property rights in Cordex Forms, including the software, documentation, updates and branding, are and remain owned by us or our licensors. Except for the limited licence granted to you, no rights are transferred.
Warranties
Our goods come with guarantees that cannot be excluded under the Australian Consumer Law. Subject to those non-excludable rights and to the extent permitted by law, Cordex Forms is provided "as is" and "as available"; we do not warrant that it will be uninterrupted or error-free, or compatible with every environment or configuration.
Liability
To the maximum extent permitted by law and subject to non-excludable consumer guarantees, we are not liable for indirect or consequential loss or for loss of profits, revenue, data, goodwill or anticipated savings, and our total aggregate liability in connection with Cordex Forms is limited to the fees you paid for the relevant licence in the twelve (12) months before the event giving rise to the liability. Where permitted, our liability for failing to meet a consumer guarantee is limited to re-supplying or paying the cost of re-supplying the software. Our professional liability is further limited by a scheme approved under Professional Standards Legislation.
Termination
We may suspend or terminate your licence if you materially breach these Cordex Forms Terms (including non-payment or exceeding entitlements) and fail to remedy a remediable breach within a reasonable time after notice, or immediately where required to protect our systems or comply with law. On termination you must stop using Cordex Forms. Provisions that by their nature survive (intellectual property, restrictions, liability, governing law) continue after termination.
Changes
We may update these Cordex Forms Terms from time to time. The current version is posted on this page with a revised "Last updated" date; material changes take effect when posted or on any later date we specify. Continued use after changes take effect constitutes acceptance.
Governing law
These Cordex Forms Terms are governed by the laws of Queensland, Australia, and the parties submit to the non-exclusive jurisdiction of the courts of Queensland and the Commonwealth of Australia.
